Preventative Health Program

 

Description: The Preventative Health Program contains many different aspects, with the goal of promoting health and prevention of primary, secondary, and tertiary diseases. Our team practices a wholistic approach with the patient and their goals at the centre of our care.

The Preventative Health Program is targeted to all ages, from birth through senior years. It is intended to help ensure the optimal health to all patients who access our services.  

 

Services and Activities offered as part of this program:

  • Immunizations and Injections: Chart review to ensure immunizations are up to date, providing immunizations as required, as well as doctor-prescribed injections.
  • Women and Child Health Services: Well-baby/Well-child health checkups according to Ministry of Ontario guidelines, counselling and patient centred education provided to women throughout the different stages of life, pre-natal and post-natal care, and breastfeeding education and support.
  • Postpartum Services: Mental Health screening is completed for all patients who deliver a baby to rule out postpartum depression and offer assistance if concerns are discovered around depression.
  • Cancer Screening Services: Offering preventative health services to adult patients as per the Ontario Cancer Screening guidlines, including Pap tests, FIT tests, and Mammograms referrals.
  • Nutrition Services: Individual nutrition counselling for a wide range of health concerns, including introduction to solids for infants, weight management, cholesterol, pre-diabetes, and gastro-intestinal issues.
  • Hospital Discharge Phone Calls: Following discharge from hospital, all patients will receive a phone call from an RN to check in and review medication changes, specialist follow-up, testing follow-up, and how the patient is managing at home. Will help arrange in-home services, if needed.
  •  Occupational Therapy Services: Home safety assessments, balance and functional mobility assessment, education on transfers and safety equipment, as well as disability assessments, and facilitating ODSP and insurance claims.
  • Pharmacy Services: Medication review and assistance with polypharmacy (multiple medications). 
  • Chiropodist: Foot pain, care of complex warts and corns, ingrown toenails, and diabetic foot care.
