Bone Health Program

 

Description: Our Nurse Practitioner, Pharmacist, Dietitian, and Occupational Therapist participate in this program to prevent illness and disability related to arthritis, rheumatic conditions, osteoporosis, and pain conditions in patients. 

 

Services and Activities offered as part of this program:

  • Falls risk/mobility assessments: to assess patient’s risk of falling, maintain and encourage independence, and prevent falls leading to injury and death.
  • Bone Health Education Series: to provide education about osteoporosis and prevention tips about maintaining bone health through diet, exercise, and medications/supplements.
  • Pharmaceutical medication review: to assess interactions and side effects and provides recommendations for additional/alternative therapies.
