Team Members

Family Health Team Model

Family Health Teams are primary health care organizations that include a team of family physicians, nurse practitioners, registered nurses, social workers, dietitians, and other professionals who work together to provide primary health care for their community. They ensure that people receive the care they need in their communities, as each team is set-up based on local health and community needs. They are governed by a Board of Directors including a mix of Community members and Physicians. The Welland McMaster Family Health Team was established in 2011 and consist of the following members.

Teaching at the Clinic

The Welland McMaster Family Health Team is affiliated with McMaster University’s Michael DeGroote School of Medicine. We are a designated teaching facility and as such we welcome medical learners from all disciplines to our clinic. This includes, but is not limited to: medical students; nursing students; and resident physicians. All learners will be safely supervised according to their level of training and we hope that you will enjoy being involved in their education. Please note that you do have the option to decline their participation in your care.
