Bone Health Program

 

Description: To prevent illness and disability related to arthritis, rheumatic conditions, osteoporosis, and pain conditions in patients.

 

Services and Activities offered as part of this program:

  • Falls risk/mobility assessments: to assess patient’s risk of falling, maintain and encourage independence, and prevent falls leading to injury and death.
  • Group Nordic Pole Walking: to increase strength and balance to reduce risk of falls, increase physical activity and physical function, and reduce social isolation.
  • Gentle, Chair, and Restorative Yoga: to reduce chronic pain, relax mind and body, enhance mood, and improve well-being.
  • Pharmaceutical medication review: to assess interactions and side effects and provides recommendations for additional/alternative therapies.
