Family Health Team Model
The Welland McMaster Family Health Team was established in 2011. Family Health Teams are healthcare organizations that include a team of Family Physicians, Nurse Practioners, Registered Nurses, and other healthcare professionals who work together to provide primary care services to patients in their community. We serve patients who are enrolled with the 10 physicians of the Welland Pelham Family Health Organization. Our main patient population is located in the Welland and Pelham areas. Teaching at the Clinic
The Welland McMaster Family Health Team is affiliated with McMaster University’s Michael DeGroote School of Medicine. We are a designated teaching facility and as such we welcome medical learners from all disciplines to our clinic. This includes, but is not limited to: medical students; nursing students; and resident physicians. All learners will be safely supervised according to their level of training and we hope that you will enjoy being involved in their education. Please note that you do have the option to decline their participation in your care.
Late Arrivals and Missed Appointments
We are constantly striving to remain on time for appointments. Therefore, if you arrive late for your appointment, you may be asked to reschedule. Please provide us with at least 24 hours’ notice if you are unable to attend your appointment so that another patient can be booked into that time slot. Missed appointments will result in a service charge.
Bill 168 (Ontario’s Occupational Health and Safety Act) states that harassment or verbal abuse in the workplace will not be tolerated. Therefore, please treat all staff with respect or you will be asked to leave.
