At the Welland McMaster Family Health Team your health is our priority. Helping you to manage your chronic disease makes up a large part of the services we offer. Learning about your disease and how best to self-manage it will help you to improve your quality of living.

Groups, Programs and Workshops

We offer many programs, groups, and workshops to our patients, all free of charge (parking costs still apply). For additional information about dates and locations, or to register, contact FHT Reception at 905-734-9699 ext. 3

COPD Clinic

Do you suffer from a persistent cough? Does performing daily activities leave you short of breath? You may have Chronic Obstructive Pulmonary Disease (COPD). Talk to your doctor or nurse practitioner about taking a free spirometry test with our Pulmonary Technician, who will also offer you counselling and tips on managing your COPD for improved quality of living.


Do you need guidance with the management of your Type II Diabetes? Ask your doctor about a referral to see the Diabetic Team, which consists of a Diabetes-specialized Registered Nurse and a Registered Diabetic Dietitian. They can help you set lifestyle-change goals, understand your medications, and can answer your Diabetes-related questions.

Heart Health Program

Do you have high blood pressure? Is your doctor concerned about your cholesterol levels but you want to learn more before you start medication? This program is for patients with Hypertension and/or High Cholesterol. The Registered Dietitian, Registered Nurse and Pharmacist will deliver an information workshop, as well as individual appointments for lifestyle-change goal setting, blood pressure monitoring, and medication information and review.


This program is for patients who are at risk of developing Type 2 Diabetes. Your doctor will refer you to an appointment with the Registered Dietitian to discuss diet and lifestyle changes that will help prevent the onset of Diabetes. A 6-month follow-up appointment will help ensure you have reduced your risk, or help guide you back on course.
